A surgical procedure that I have been performing for over 30 years is Total Joint Replacement for the first metatarsal-phalangeal joint in the foot. This procedure is performed when the cartilage has worn down and there is pain in the joint. When this occurs in the foot, not only does walking become painful, but actions such as squatting or rising up on tip-toes becomes extremely uncomfortable. There is usually limited motion at the joint, and there may be spurring or enlargement causing a noticeable bump on the top of the great toe joint.

The metatarsal-phalangeal joint replacement surgery takes about 1 hour and is done as an outpatient procedure under a combination local and sedation anesthetic. The majority of patients return to wearing a tennis shoe within 3-4 weeks and are comfortable in performing most normal day-to-day activities at that time. Range of motion exercises are started shortly after surgery, and occasionally physical therapy is instituted as well as a Dynamic Motion Device to help increase range of motion and strength. Full recovery can take anywhere between 2 - 6 months.
